The Socket pictures shown in the Socket Guide (see menu at right) can be used to help identify a bulb socket you need to replace.  You will also need to provide additional information to help us identify the correct socket your equipment uses.  Below is also a form which you can print out and fill in using the information on Socket Guide page.  You can either fax the completed form to us or call a member of our Customer Service staff at extension 100 with the information.  We'll identify the socket and contact you with the information within 24 hours of your request.
